If you are a new patient with Grantsville Family Dentistry you
can save time by filling out the forms before you come in. Please fill out
the forms on your computer and email them to
info@grantsvillefamilydental.com.
Form instructions:
- Please click the "Download Forms" button.
- A new window should open up as an Acrobat Reader form.
- Fill out the forms until complete.
- Click the icon (picture) of the Floppy Disk in the upper left portion of the
form.
- A new "save a copy" window will appear and then rename the file "gdf
forms".
- Then click Save.
- Then open your email and click the attach form and browse for the saved "gfd
forms" file and send it to
reception@grantsvillefamilydental.com.
